MEDICAL

Full-time (24+hours) employees are eligible for medical insurance on the 1st of the month following date of hire. Coverage includes physician visits, hospital services, mental health care, prescription drug benefits, and preventive care. UMC pays 75% of Base plan employee & dependent premium.

DENTAL

Full-time (24+hours) employees are eligible for dental insurance on the 1st of the month following date of hire. Coverage includes preventive, routine and major services. UMC pays 75% of the employee & dependent premium.

VISION

Full-time (24+hours) employees are eligible for vision insurance on the 1st of the month following date of hire. Coverage includes one routine annual eye exam, lenses every 12 months, or frames every 24 months (subject to plan co-pay and maximum limits)

UNUM LIFE & ACCIDENTAL DEATH AND DISMEMBERMENT (AD&D)

Full-time (24+hours) employees are eligible on the 1st of the month following date of hire. Life insurance and AD&D all provide 1x annual salary up to $100,000. 100% company-paid. Employees have the option to purchase additional coverage.

UNUM SHORT TERM AND LONG TERM DISABILITY INSURANCE

Full-time (24+hours) employees are eligible on the 1st of the month following date of hire. Benefits are paid at 60% of monthly base earnings after 7 days of disability for STD and 180 days of disability for LTD. 100% company-paid.

HEALTH CLUB REIMBURSEMENT

Employees will be reimbursed for up to $250 per year for a fitness program. A "fitness" program can include health club memberships, exercise, and fitness programs.

HOLIDAYS

Full-time employees will receive a paid day off for New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Day after Thanksgiving, Christmas Day, and one Floating Holiday.
